UnitedHealth Group’s Heart of Hospice in Fort Smith, AR seeks a Registered Nurse Case Manager to oversee the Plan of Care for each hospice patient, collaborating with professionals to implement strategies promoting physical, psychosocial, and spiritual well-being. The role requires assessing needs and ensuring timely care within all home settings.

In this position, you will evaluate performance, document clinical progress, and support families facing end-of-life care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license or access to reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care and documents goals, interventions, and responses.
• Collaborates with patient/family, attending physician and IDG members daily to provide care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
