Job summary

Heart of Hospice in Fort Smith, AR, part of the LHC Group within Optum, is seeking an RN Case Manager to oversee the Plan of Care for hospice patients and collaborate with the care team to promote holistic well-being.

Requires RN licensure in the state, a valid driver's license and reliable transportation; CPR certification or ability to obtain within 90 days. Preferred candidates have 1+ year clinical experience. Includes comprehensive benefits for full-time employment.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions and responses.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs and supervises patient/family in self-care as appropriate.
• Maintains accurate clinical notes and adapts care for diverse patient needs.
