Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to reliable transportation or public transit.

Responsibilities

• Make the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identify the patient's/family's physical, psychosocial, and environmental needs and reassess as needed, at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with the patient/family, attending physician and IDG members in providing daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.
• Function in any home situation regardless of demographics.
• Be recognized for performance and development opportunities within a challenging environment.
