Job summary

Northern Arizona Hospice, part of the Optum family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will evaluate and adjust care, collaborate with the team, and support patients and families through the hospice journey.

Responsibilities include initial nursing evaluation within 48 hours of referral, coordinating the plan of care, documenting progress, and ensuring quality care across settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family needs and re-assesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care and documents progress.
