Job summary
Hearth Hospice is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team serving Hamilton County. Schedule: Mon-Fri 8am-5pm, performing hands-on hospice care, coordinating services, and guiding patients and families through their hospice journey with compassion and clinical excellence.

Qualifications
- Hold current unencumbered RN license.
- Minimum one year RN experience in med-surg/acute care; hospice exp is a plus.
- Computer proficiency including EMR background.
- CPR certification may be required by state.
- Current driver's license, vehicle insurance, and reliable transportation.
Responsibilities
- Respond to referrals with care and expertise, addressing inquiries and requests with compassion.
- Collaborate with Clinical Director to assess eligibility for hospice services.
- Provide direct care coordination to maintain patient comfort and quality of life.
- Deliver nursing care rooted in proven principles to ease pain and provide solace.
- Educate and advocate for patients and families to create personalized care plans.
- Share call duties as part of a team when needed.
