Job summary

Intermountain Health seeks a compassionate Registered Nurse to join our hospice care team in the home setting. You will assess, plan, implement, and evaluate patient-centered end-of-life care, coordinate with an interdisciplinary team, and educate families to comfort and dignity.

Requirements include an active RN license in Utah, BSN, and BLS; a valid driver’s license; and strong communication. We offer a stable schedule of three 12-hour shifts each weekend, competitive pay, and opportunities

Qualifications

• Current RN license in state of practice.
• BSN degree required; BSN must be obtained within four years if less than 12 months of experience.
• BLS certification for healthcare providers.
• Valid driver’s license with reliable transportation.
• Effective communication with patients, families, and healthcare teams.

Responsibilities

• Provide holistic, patient-centered hospice care in the home setting.
• Assess, plan, implement, and evaluate individualized care plans.
• Educate patients and families on care and symptom management.
• Coordinate with interdisciplinary healthcare team for seamless care.
