Job summary

Qualifications

• RN license in state of practice with active status.
• BSN degree required; HS Diploma not acceptable for this role.
• BLS certification for healthcare providers.
• One year RN experience in Acute Care, Home Health, or Hospice preferred.

Responsibilities

• Assess, plan, implement, and evaluate individualized hospice care plans.
• Provide holistic, patient-centered care in the home setting.
• Coordinate with an interdisciplinary team and support families.
