Job summary

Allay Hospice in Wisconsin is seeking a Registered Nurse Case Manager to join our hospice team. You will deliver patient-centered, goal-driven nursing care, develop care plans, and collaborate with social workers, chaplains, aides, and physicians to ensure seamless, compassionate support across home settings.

The role features a 1:1 patient focus, strong interdisciplinary teamwork, leadership support, and an after-hours triage network.

Qualifications

• Current RN licensure in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care and documentation standards.

Responsibilities

• Assess/monitor patient needs (physical, emotional, psychological).
• Create and adjust hospice care plans aligned with patient goals.
• Direct nursing care: medications, treatments, interventions.
• Provide pain and symptom management and family education.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate, timely documentation and scheduling.
