Job summary

Allay Hospice is seeking a Registered Nurse Case Manager to join our hospice team in the Brookfield, Wisconsin area. You will coordinate patient care, support families, and work with an interdisciplinary team to deliver compassionate, patient‑centered services. A $5,000 sign‑on bonus is offered for eligible candidates.

The role features a full-time schedule, Monday–Friday 8AM–5PM, and travel across Waukesha and Milwaukee Counties. Prior hospice experience and Wisconsin licensure are preferred.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in Wisconsin
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with a vehicle that meets state requirements and is in good working order

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs
• Create hospice care plans aligned with patients' wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patients’ families and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on‑call rotation as required by local branch
