Job summary

Allay Hospice is seeking a passionate Hospice RN Case Manager to join our team in Wisconsin. You will deliver patient-centered care, develop care plans, and work with an interdisciplinary team to ensure comfort and goals are met.

We offer strong nurse support, a collaborative environment, and opportunities for professional growth. Schedule is full-time, Monday–Friday, with patch-based travel within the territory.

Qualifications

• Graduate of an accredited nursing school with current WI licensure.
• One year nursing experience required, 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a licensed driver with insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
