Job summary

Allay Hospice is seeking a passionate Hospice RN Case Manager to join our team. You will provide expert, patient-centered care at a critical time in people’s lives, working within a supportive interdisciplinary team and utilizing Homecare Homebase (HCHB) EMR for documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
