Job summary

Allay Hospice, part of BrightSpring Health Services, is seeking a full-time RN Case Manager to provide comprehensive hospice nursing in the Brookfield area with travel across Waukesha and Milwaukee counties. You will work Monday–Friday, 8 AM–5 PM, supported by an experienced interdisciplinary team and an after-hours triage system to maintain care continuity.

Qualifications

• Current state nursing license to practice.
• 1 year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on‑call rotation as required by local branch.
