Job summary

Allay Hospice is seeking a dedicated Hospice RN Case Manager to join our team in Brookfield, WI. You will provide compassionate, patient-centered care with lower patient-to-nurse ratios and strong interdisciplinary support, coordinating with Social Work, Chaplaincy, Aides and providers.

This full-time role offers a $5,000 sign-on bonus and comprehensive benefits including medical, dental, vision, 401(k), generous PTO, mileage reimbursement, and tuition assistance.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Must be a licensed driver with an automobile that is insured and in good working order

Responsibilities

• Assess patient needs and monitor physical, emotional, and psychological status
• Create hospice care plans aligned with patient wishes and goals
• Direct nursing care: administer medications, treatments and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required
