IHA in Mason City, IA is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered care to hospice patients and families. The role includes assessment, care planning, symptom management, and coordinating services to ensure quality end-of-life care.

The position requires an accredited nursing program graduate, current RN license in IA, CPR certification, and at least one year of acute care experience. A sign-on bonus of $10,000 is offered.

Qualifications

• Strong communication and relationship-building skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Manage a hospice patient caseload and serve as primary case manager.
• Complete comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains and other team members.
• Manage pain and symptoms using evidence-based hospice practices.
• Educate patients and families on disease progression and end-of-life care.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and care conferences.
• Supervise Hospice Aides and LPNs and oversight of care plans.
• Support patients and families through grief and emotional transitions.
