Job summary

Queen City Hospice is seeking a Registered Nurse (RN) to join our team in Southwest Ohio. You will manage patient caseloads, perform full nursing assessments, create and implement care plans, and oversee care delivery with HHAs to ensure high-quality, compassionate end-of-life care.

The role emphasizes holistic patient care, collaboration with physicians, social workers, and chaplains, and thorough documentation in compliance with standards.

Qualifications

• Active RN license in Ohio or compact state.
• Graduate of an accredited nursing program.
• Current CPR and First Aid certification.
• Knowledge of quality assurance and hospice care standards.
• Valid driver’s license and auto insurance.

Responsibilities

• Manage a patient caseload and lead the full nursing process assessment admission care planning implementation and evaluation.
• Deliver holistic patient care addressing physical emotional psychological social and spiritual needs.
• Collaborate with the interdisciplinary care team including physicians social workers and chaplains.
• Coordinate care with attending physicians and external providers.
• Conduct routine patient visits and update individualized plans of care.
• Provide end of life care including clinical pronouncement and family support.
• Maintain accurate timely documentation in compliance with state federal and company standards.
• Supervise and oversee care delivery by Home Health Aides (HHAs).
