Job summary

Capital City Hospice in Ohio is seeking an RN Case Manager – Hospice to join a caring team. You will serve as the primary RN for patient and family caseload, perform assessments to determine eligibility, lead admissions, and develop individualized plans of care.

The role emphasizes regular patient visits, coordination with physicians and partners, and compassionate support for families, with a focus on quality, documentation, and compliant care delivery.

Qualifications

  • Graduate of an accredited registered nursing program.
  • Current RN license in the state of practice.
  • Current CPR and First Aid certification.
  • Knowledge of quality assessment and assurance practices.
  • Strong clinical judgment and ability to make independent decisions.
  • Compassionate communication skills to support patients and families.
  • Valid driver’s license and reliable, insured vehicle.

Responsibilities

  • Serve as the primary RN for your patient and family caseload
  • Complete assessments and determine hospice eligibility
  • Lead the admission process and develop individualized plans of care
  • Evaluate physical, emotional, social, and spiritual needs and coordinate interventions with the interdisciplinary team
  • Communicate with attending physicians, team members, and partner agencies to ensure seamless care
  • Conduct regular patient visits to reassess needs and update care plans
  • Perform clinical pronouncements of death and provide compassionate support to families
  • Maintain accurate, timely documentation, care plans, and progress notes
  • Supervise and document care provided by Home Health Aides per federal, state, and company guidelines

Schedule
  • Monday–Friday, 8 a.m.–5 p.m.
  • Only 2 holiday on-call shifts per year
