Job summary

Queen City Hospice is seeking a dedicated RN Case Manager to join our Ohio team. You will manage a patient caseload and lead the full nursing process, delivering holistic care and coordinating with the interdisciplinary team to ensure quality, patient‑centered outcomes.

Join us to support end‑of‑life care, maintain clear plans of care, and mentor Home Health Aides while upholding high standards of documentation and compliance.

Qualifications

• Active RN license in Ohio (or compact state) and eligible to practice.
• Graduate of an accredited nursing program.
• Current CPR and First Aid certification.
• Knowledge of quality assurance and hospice care standards.
• Driver's license and auto insurance.

Responsibilities

• Manage a patient caseload and lead the full nursing process from assessment to evaluation.
• Deliver holistic patient care addressing physical, emotional, social and spiritual needs.
• Collaborate with the interdisciplinary care team including physicians, social workers and chaplains.
• Coordinate care with attending physicians and external providers.
• Conduct routine patient visits and update plans of care.
• Provide end‑of‑life care and family support.
• Maintain accurate documentation in compliance with standards.
• Supervise and oversee care delivery by HHAs.

Key Responsibilities

As a Hospice RN Case Manager, you will:

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
• Deliver holistic patient care, addressing physical, emotional, psychological, social, and spiritual needs
• Collaborate with the interdisciplinary care team, including physicians, social workers, and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update individualized plans of care
• Provide end-of-life care, including clinical pronouncement and family support
• Maintain accurate, timely documentation in compliance with state, federal, and company standards
• Supervise and oversee care delivery by Home Health Aides (HHAs)

Qualifications

• Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of quality assurance and hospice care standards
• Valid driver's license and auto insurance
