Job summary

Ecumen Hospice St Cloud is hiring a Registered Nurse RN for casual/PRN shifts in the Litchfield and South territories, with on-call coverage spanning the entire area. The RN triages calls, visits patients with changing conditions, and supports admissions as needed.

Candidates must be a graduate of an accredited RN program and hold state RN licensure. A BSN is preferred. Local travel and a valid driver’s license are required. Benefits include PTO and EAP options.

Qualifications

• Graduate of an accredited Registered Nursing program.
• 1 year nursing experience in post-acute care setting.
• Must be licensed as a Registered Nurse within the state in which working.
• Valid Driver's License, proof of insurance and satisfactory motor vehicle record.
• Ability to communicate effectively both verbal and written.
• Proficient with Microsoft Office Suite.
• Follow all safety rules, regulations, policies and procedures.

Responsibilities

• Receives and responds to phone calls during assigned shifts as well as making visits to patients with changing conditions and completing weekend admissions.
• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Assesses and evaluates patient's status by writing and initiating plan of care and re-evaluating needs.
• Develops a care plan based on nursing diagnosis with patient and family input.
• Administers medications and treatments as prescribed and records changes/outcomes.
• Initiates the plan of care and revises as patient needs change; communicates with physician.
• Provides health care instructions to the patient as appropriate.

Schedule

Casual / PRN / Pick up shifts as they become available. Primary Focus when working shifts - Litchfield and South areas - when on-call would support the entire territory.

Pay

The targeted pay range for this job is $38.50 - $43.50 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.

Essential Job Responsibilities

1. Receives and responds to phone calls during assigned shifts as well as making visits to patients with changing conditions and completing weekend admissions.
2. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness.
3. Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
5. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Records pain and/or symptom management changes/outcomes as appropriate.
6. Initiates the plan of care and makes necessary revisions as patient status and needs change. Communicates with the physician regarding patient needs and reports changes in the patient's condition; seeks physicians' orders as required.
7. Provides health care instructions to the patient as appropriate per assessment and plan.

Registered Nurse HSP Minimum Required Qualifications

• Graduate of an accredited Registered Nursing program
• 1 year of nursing experience in post-acute care setting
• Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.

• Ability to communicate effectively both verbal and written
• Proficient with Microsoft Office Suite
• Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.

Preferred Qualifications

• Bachelor of Science degree in Nursing
• One year nursing experience in Hospice
