Job summary

Athena Home Health & Hospice seeks a Hospice RN Case Manager on a per diem basis to plan, organize, and direct hospice care services in patients' homes and communities in Connecticut.

Responsibilities include supervising aides, conducting comprehensive assessments, creating care plans with palliative actions, counseling families, and coordinating care as Case Manager under the Clinical Supervisor.

Qualifications

• Graduate of an accredited school of nursing.
• Connecticut RN license, active and unencumbered.
• Bachelor's degree, hospice care experience preferred.
• 1–2 years recent acute care + 1–2 years in home care.

• Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload.

As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations
• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate
• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements:

• Graduate of an accredited school of nursing
• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
• Active and unencumbered RN license in Connecticut
• CPR certification
• Bachelor's degree, with one (1) year of hospice care experience preferred
