Job summary

Athena Home Health & Hospice is seeking a compassionate Hospice RN Case Manager to plan, organize, and direct home-based hospice care for patients in Litchfield and western Hartford counties. This salaried role offers weekly pay up to $49/hour depending on experience.

The position requires an RN license in CT, CPR, and hospice care experience. Shift is Monday–Friday, 8:00 a.m.–4:30 p.m., with opportunities for career advancement and benefits.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care experience in an institutional setting and 1–2 years in home care.
• Active and unencumbered Connecticut RN license.
• CPR certification.
• Bachelor's degree with hospice care experience preferred.

Responsibilities

• Supervise hospice aides and provide professional nursing care using the nursing process.
• Complete comprehensive assessments of patient and family to determine hospice needs and provide re-evaluations.
• Develop and implement care plans based on nursing diagnosis and palliative actions.
• Counsel patients and families in meeting nursing and related needs and provide health care instructions.
• Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseload.

Hospice RN Case Manager

Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager. As a vital member of our hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

This position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation.

Responsibilities

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations
• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate
• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements

• Graduate of an accredited school of nursing
• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
• Active and unencumbered RN license in Connecticut
• CPR certification
• Bachelor's degree, with one (1) year of hospice care experience preferred
