Athena Home Health & Hospice in Southern New England seeks a compassionate Hospice RN Case Manager to plan, organize, and direct home-based hospice care.

The role covers Litchfield County and western Hartford County, is salary-based, with weekly pay up to $49 per hour based on experience. Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m., relocation not required.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in Connecticut; CPR certification.
• 1–2 years acute care in institutional setting and 1–2 years home care.
• Bachelor's degree preferred with hospice exposure.

Responsibilities

• Supervise hospice aides and provide professional nursing care.
• Complete comprehensive patient/family assessments for hospice needs.
• Develop and implement care plans with goals from nursing diagnoses.
• Counsel patients and families and provide health instructions.
• Act as Case Manager coordinating care for assigned caseload.

Hospice RN Case Manager

Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager. As a vital member of our hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

This position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation.

Responsibilities

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations
• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate
• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements

• Graduate of an accredited school of nursing
• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
• Active and unencumbered RN license in Connecticut
• CPR certification
• Bachelor's degree, with one (1) year of hospice care experience preferred
