Job summary

Athena Health Care Systems in Connecticut is seeking a Hospice RN Case Manager to provide nursing care in home health and hospice settings on a per diem basis. You will plan, organize, and direct hospice care services to meet patient and family needs in homes and communities.

Responsibilities include supervising aides, conducting comprehensive assessments, developing care plans with palliative actions, counseling patients and families, and serving as Case Manager for assigned caseload to ensure

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years acute care in institutional setting and 1–2 years home care.
• Active and unencumbered RN license in Connecticut.
• CPR certification.
• Bachelor's degree with Hospice care experience preferred.

Hospice RN Case Manager - per diem up to $49 an hour

As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process

• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations

• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions

• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate

• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements:

• Graduate of an accredited school of nursing

• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care

• Active and unencumbered RN license in Connecticut

• CPR certification

• Bachelor's degree, with one (1) year of hospice care experience preferred
