Job summary

Athena Home Health & Hospice is seeking a compassionate Hospice RN Case Manager to coordinate in-home hospice care across Litchfield County and western Hartford County in Connecticut. The role is on-the-road, salaried with weekly pay, and offers opportunities to supervise care teams and develop care plans centered on comfort and patient-family support.

Requirements include an accredited nursing degree, Connecticut RN license, CPR certification, and 1–2 years of acute care and hospice/home care

Qualifications

• Connecticut RN license currently active.

• 1–2 years acute care experience; 1–2 years home care experience preferred.

Responsibilities

• Supervise hospice aides and provide professional nursing care using the nursing process.
• Complete comprehensive patient and family assessments to determine hospice needs and re-evaluate regularly.
• Develop and implement care plans based on nursing diagnosis and palliative actions.
• Counsel patients and families in nursing needs and provide health care instructions.
• Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for caseload.

Hospice RN Case Manager

Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager. As a vital member of our hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

This position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation.

Responsibilities

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations
• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate
• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements

• Graduate of an accredited school of nursing
• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
• Active and unencumbered RN license in Connecticut
• CPR certification
• Bachelor's degree, with one (1) year of hospice care experience preferred
