Job summary

Athena Home Health & Hospice is seeking a Hospice RN Case Manager to plan, organize, and direct hospice care in patients' homes and communities. You will supervise aides, perform comprehensive assessments, and develop care plans aligned with nursing diagnoses and palliative actions.

You will also counsel patients and families, ensuring understanding of nursing needs, and serve as Case Manager for assigned caseload under the Clinical Supervisor.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years acute care experience; 1–2 years home care experience.
• Active RN license in Connecticut and CPR certification.
• Bachelor's degree; hospice care experience preferred.

• Complete comprehensive patient/family assessments and regular re-evaluations.

Hospice RN Case Manager - per diem

As a vital member of our Athena Home Health & Hospice care team, you will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process

• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations

• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions

• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate

• Act as Case Manager when assigned by Clinical Supervisor, assuming responsibility to coordinate patient care for assigned caseload

Requirements:

• Graduate of an accredited school of nursing

• One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care

• Active and unencumbered RN license in Connecticut

• CPR certification

• Bachelor's degree, with one (1) year of hospice care experience preferred
