Job summary

Day City Hospice in Ohio seeks a compassionate Registered Nurse (RN) to join our team for a Monday–Friday day shift in Dayton and surrounding areas. The role emphasizes low patient caseloads, strong clinical support, and a community-focused, patient-centered approach.

As Hospice RN Case Manager you will assess, plan, implement, and evaluate care, collaborate with an interdisciplinary team, provide end‑of‑life support to patients and families, and ensure accurate documentation.

Qualifications

• Active RN license in Ohio (or compact state).
• Graduate of an accredited nursing program.
• Current CPR and First Aid certification.
• Knowledge of quality assurance and hospice care standards.
• Valid driver’s license and auto insurance.

Responsibilities

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation.
• Deliver holistic patient care addressing physical, emotional, psychological, social, and spiritual needs.
• Collaborate with interdisciplinary care team, including physicians, social workers, and chaplains.
• Coordinate care with attending physicians and external providers.
• Conduct routine patient visits and update individualized plans of care.
• Provide end-of-life care, including clinical pronouncement and family support.
• Maintain accurate, timely documentation in compliance with state, federal, and company standards.
• Supervise and oversee care delivery by Home Health Aides (HHAs).

Key Responsibilities
