Job summary

Augusta Health in Virginia’s Shenandoah Valley seeks an experienced Registered Nurse Care Manager for hospice services. You will coordinate care with the Interdisciplinary Team, patients and families, and ensure quality end-of-life care across home settings.

Responsibilities include medication management, patient education, and documentation. Applicants must have an active RN license in VA or a compact state, 1+ year of RN experience, CPR/BLS, and a valid driver's license.

Qualifications

• Education: Nursing Diploma and/or Associate Degree in Nursing.
• Licensure: Current and valid Registered Nurse license in Virginia or a compact state.
• CPR-Basic Life Support Healthcare Provider (BLS-HCP).
• Experience: Minimum of one year of RN experience.
• Driver's license: Yes; driving Augusta Health vehicle requires MVR approval.

Responsibilities

• Coordinate plan of care for hospice patients and educate patient/family.
• Lead the interdisciplinary team to ensure quality, age-appropriate care.
• Document nursing interventions, patient responses, and outcomes.
• Maintain infection control, safety, and compliance with CMS/ACHC standards.
