Job summary

Augusta Health in Fishersville, VA, is looking for a Hospice RN Case Manager (Care Manager) on a part-time, on-site basis. You will coordinate the plan of care for hospice patients in their homes, guiding the Interdisciplinary Team and educating patients and families on end-of-life care.

Key qualifications include an RN license, BLS, and a minimum of one year of RN experience. A valid driver's license is required.

Qualifications

• Education: Nursing Diploma and/or ADN.
• RN license in Virginia or a state in compact with Virginia.
• CPR-BLS-HCP certification.
• Minimum 1 year RN experience.
• Valid driver’s license and ability to drive Augusta Health vehicle.
• Eligible to work in the United States.

Responsibilities

• Administers nursing care per medical plan and standards.
• Coordinate care with the Interdisciplinary Team, patients, families, and caregivers.
• Educates patient and family on disease process, end-of-life care, and self-care techniques.
• Assess acuity and plan of care based on age-specific needs.
• Document interventions and patient outcomes.
• Prepare clinical and progress notes ensuring timely documentation.
