Job summary

Trinity Hospice is seeking a Patient Care Manager (RN) to lead the Interdisciplinary Team and ensure high-quality care in patients' homes. The PCM will oversee staff, monitor staffing and productivity, and coordinate care with patients, families, and referring agencies.

The role requires an RN license, hospice experience preferred, CPR, and strong communication and computer skills. It is a full-time exempt position with a focus on quality and regulatory compliance.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current RN license in the state.
• CPR certification or ability to obtain.
• Minimum one year RN experience, hospice preferred.
• Interpersonal and communication skills required.
• Computer proficiency and EMR experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Directs and coordinates designated IDT(s); ensures continuity, quality, and safety of services in accordance with regulations.
• Promotes agency policies to maintain quality patient care.
• Supervises staff and ensures timely completion of IDT notes, visits, and payroll reviews.
• Reviews admissions, discharges, medications, supplies, and notes for appropriateness of care.
• Participates in QAPI activities.
• Collaborates with the Director of Nursing/Administrator to ensure compliance and service quality.
