Job summary

Angels Care Hospice is seeking a Registered Nurse Patient Care Manager (PCM) to coordinate and manage patient care across the hospice team. This role emphasizes leadership, case management, and adherence to hospice standards while supporting patient and family needs.

Qualifications

• Registered nurse with current licensure to practice in the state.
• BSN degree preferred.
• Previous experience in hospice and/or home care setting.
• 2 years of management or supervisory experience.
• Complies with professional standards and practice.
• Excellent observation, nursing judgment, and communication skills.
• Understands hospice philosophy and death/dying issues.
• Knowledge of pain and symptom management.

Responsibilities

• Receives case referrals and reviews available patient information to determine hospice care needs.
• Evaluates each case through home visits, conferences, record review and clinician-provided services.
• Reviews patient diagnosis, prognosis, medications, procedures, and clinical course.
• Assists clinicians in establishing immediate and long-term goals, setting priorities, and developing care plans.
• Attends case conference meetings to coordinate care and interdisciplinary involvement.
• Conducts quarterly record reviews and communicates findings to Clinical Director and hospice personnel.
• Assesses community health needs and assists in developing plans/programs to meet needs.
• Participates in public relations and community activities.
• Promotes customer service orientation for hospice personnel.
• Participates in quality assessment and performance improvement teams and activities.
• Performs other duties delegated by the Executive.
