Qualifications

• Registered Nurse with current state licensure.
• CPR certification mandatory.
• Two years of management or supervisory experience in a home care setting.
• Proven ability to work effectively within an interdisciplinary team.
• Licensed driver with reliable insured vehicle.
• Comprehensive knowledge of professional standards, Medicare guidelines, and regulatory requirements.

Responsibilities

• Coordinate and communicate care needs reported by case managers.
• Maintain updated schedules and ensure continuity of care throughout the patient’s episode.
• Lead Case Conferences, facilitating follow-up actions from meetings and addressing care concerns.
• Mentor and coach clinicians with monthly one‑on‑one sessions for development and accountability.
• Address team attrition, serving as the primary contact for support and resolution of personnel issues.
• Oversee team operations including scheduling, visit assignments, and timely completion of care visits.
• Manage site operations in the absence of senior leadership.
• Handle clinical, administrative, and record‑keeping tasks efficiently.
• Respond promptly to referral inquiries and participate in on‑call nursing duties.
