Job summary

Community Healthcare of Texas is seeking a Patient Care Manager to oversee clinical operations and direct patient care across the hospice team in Waco, TX. The PCM will coordinate with clinicians, case managers, and leadership to ensure high-quality, compliant care.

Responsibilities include staff scheduling, patient care planning, case conferences, and QA activities while maintaining regulatory standards and promoting the CHOT mission of compassionate, patient-centered service.

Qualifications

• Requires a college degree in nursing; BSN strongly preferred.
• Requires 2+ years of hospice experience and supervisory experience.
• Licensed RN in TX and valid TX Driver's License.
• Proficient computer skills; Microsoft Office suite experience preferred.
• Good communication skills and ability to travel independently.

Responsibilities

• Oversee the admission process for new patients and confer with the team for guidance.
• Manage direct patient care by assigning staff and scheduling for budget optimization, including PTO/on-call.
• Review patients' medical diagnoses, prognosis, medications, procedures, and clinical course.
• Assist clinicians in establishing goals, priorities, and care plans.
• Attend case conferences to coordinate care among interdisciplinary teams.
• Assist in hiring, orientation, and ongoing staff development and education programs.
• Audit patient charts and ensure compliance; participate in QA activities and regulatory audits.
