VNA Health Group is seeking a Registered Nurse (RN) to join our Hospice Baylor care team in Ocean, NJ. The role involves delivering skilled nursing services to patients in their homes, with a schedule designed for balance and flexibility.

The Baylor position covers every Saturday and Sunday, with 12-hour shifts and compensation for 30 hours. You will assess patient needs, develop individualized care plans, educate families, and coordinate with the care team.

Qualifications

• Current NJ RN license is required.
• Minimum 1 year of clinical nursing experience; home care experience preferred.
• Strong communication, clinical judgment, and critical thinking skills.
• Proficient with EMR and digital tools.
• Reliable car, valid driver’s license, and proof of auto insurance required.

Responsabilités

• Deliver skilled nursing services and coordinate care for patients in their homes.
• Evaluate patient needs, create individualized care plans, and monitor outcomes.
• Educate and empower patients and families on treatment plans and self-care strategies.
• Collaborate with physicians, case managers, and interdisciplinary team members.
• Document accurately and timely in the EMR system.
