Job summary
Beth Israel Lahey Health is seeking a Weekend Baylor Hospice RN to plan and deliver patient care over two 12-hour weekend shifts. This role emphasizes palliative care, patient/family counseling, and strong collaboration with the interdisciplinary team.
The position includes full-time benefits, 24 hours paid for 32, and a comprehensive compensation package with potential for PTO, 403(b), and tuition reimbursement. Active NH RN license and BLS required.
Qualifications
- Active NH RN license and BLS certification.
- BSN or ADN from an accredited school (preferred).
- At least 1 year of direct care nursing experience.
- OASIS or EPIC experience preferred.
- Ability to coordinate care with inter-disciplinary teams.
Responsibilities
- Manage care of patients and coordinate all services and communication between providers.
- Provide skilled nursing care and prescribed treatments in the home.
- Document services using problem-oriented records.
- Plan continuity of care with nursing team, patient, and family.
- Teach nursing care, comfort measures and health practices to patient and family.
- Instruct and supervise Hospice Aides.
