Job summary

Constellation Health Services is seeking a compassionate Registered Nurse to provide skilled nursing care to hospice patients in their homes, assisted living facilities and skilled nursing settings. You will assess patients, coordinate care with the interdisciplinary team, and document in our electronic system, including weekend and holiday coverage as needed.

The role requires an accredited RN program graduate, state RN licensure, and 1–2 years of RN experience; hospice experience is highly

Qualifications

• Graduate of an accredited RN program and RN license in the state of employment.
• At least 1–2 years of RN experience; hospice experience is strongly desired.
• Familiarity with home health and family/caregiver collaboration preferred.

Responsibilities

• Administer medications and treatments per physician's plan in patients' homes.
• Perform comprehensive assessments and coordinate the plan of care with the team.
• Observe, evaluate and report changes to the nursing supervisor and physician.
• Participate in weekly Interdisciplinary Team Meetings and document outcomes.
• Maintain documentation in the required system and timelines.
• Provide input for hospice aide evaluations and weekend/holiday coverage as needed.
