Job summary

Elara Caring is seeking an After-Hours/On-Call Registered Nurse to support hospice patients in the Toledo, OH area. You will assess needs, monitor conditions, and educate families and staff on the interdisciplinary Plan of Care.

This role involves travel between patient homes and occasional lifting in a fast-paced home health setting. Candidates must hold a current RN license and have 1+ year of nursing experience; home health experience is preferred.

Qualifications

• Current State license as a Registered Nurse.
• 1+ year of clinical nursing experience; home health/hospice preferred.
• Must be able to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assess hospice patients and families to identify care needs across physical, psychological, social, and spiritual domains.
• Monitor patient condition and communicate changes to physician and care team.
• Educate patients, families, caregivers, and staff on plan of care and hospice philosophy.
• Provide bereavement support to families and caregivers.
