Job summary

Elara Caring is seeking an After Hours/ On Call Registered Nurse to join our home health team. You will support hospice patients and families in their homes, assess needs, and collaborate with an interdisciplinary team to ensure high-quality care.

Requires RN license, 1 year clinical experience, and 50% travel. Valid driver's license and reliable transportation are essential. Join us to provide compassionate care at the right time, in the patient's home.

Qualifications

Graduate of an accredited nursing program.
Current State License as a Registered Nurse (RN).
1 year of clinical care experience.
Hospice/home health experience preferred.
50% travel required.
Valid driver’s license and reliable transportation.

Responsibilities

Assess hospice care patients and families to identify physical, psychosocial, and environmental needs.
Assess patient needs and collect data on physical, psychological, social, and spiritual factors.
Communicate significant findings to the Clinical Supervisor, physician, facility, and other personnel.
Counsel, instruct, and include patient, facility, and family in the Interdisciplinary Plan of Care.
Teach hospice philosophy and pain management to staff in alternate care settings.
Provide support at time of death and perform bereavement assessment.
