Job summary

Elara Caring is seeking an After Hours/ On Call Registered Nurse to join our hospice/home health team. This role supports patient care in the home setting, reporting to the Clinical Team Manager, and involves after-hours responsibilities with a mix of on-call duties across geographic areas.

Responsibilities include assessing patient needs, communicating with the care team, and educating staff and families on the plan of care.

Qualifications

• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Current State License as a Registered Nurse (RN).
• 1 year of clinical care experience.
• Hospice or home health experience preferred.
• Travel required up to 50%.
• Valid driver's license and reliable transportation to perform job tasks.

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Communicate significant findings to the Clinical Supervisor, physician, and care team.
• Counsel, instruct, and include patient and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain/symptom management to staff in alternate care settings.
• Provide bereavement support at time of death.
