Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse to help grow the home health industry. You will assess patients and families, monitor conditions, and educate staff across various care settings while supporting bereavement needs.

Requirements include an active RN license, a nursing degree, 1+ year of clinical nursing experience, and the ability to lift up to 50 pounds. Valid driver’s license or reliable transportation is required.

Qualifications

Current State license as a Registered Nurse.
Graduate of an accredited nursing program (Certificate, Diploma, Associates, or Baccalaureate).
1+ year of experience as a nurse in a clinical care setting.
Home health experience preferred.
Ability to lift up to 50 pounds and assist patient transfers.
Must possess a valid driver’s license or reliable transportation.
Hospice/home health experience preferred.

Responsibilities

Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
Monitor patient condition and report significant changes to the physician and care team.
Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
Provide education on hospice philosophy, pain management, and symptom management to staff in care settings.
Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
