Job summary

Elara Caring is seeking a Hospice Registered Nurse for after-hours and on-call coverage. The role emphasizes care delivered in the patient’s home, with a focus on compassionate, patient-centered hospice services.

You will operate within the hospice team to ensure high-quality care across shift times and locations. The position requires a nursing degree and an RN license, with 1 year of clinical experience; hospice/home health experience is preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current State RN license.
• Minimum 1 year clinical care experience.
• Hospice or home health experience preferred.
• Ability to travel up to 50%.

Responsibilities

• Ensure care aligns with Elara Caring's mission and Interdisciplinary Plan of Care.
• Assess hospice patients and families to identify needs documented in records and care plans.
• Communicate significant health findings to supervisors and involved staff.
• Counsel and include patient and family in care planning.
• Provide bereavement support at time of death and educate staff on hospice principles.
• Support staff across care settings with hospice pain and symptom management.
