Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse in Oklahoma to join our team delivering compassionate hospice and home health care. You’ll provide critical nursing support in patients’ homes and facilities during evenings and on-call shifts.

Responsibilities include comprehensive assessments, monitoring patient conditions, coordinating with physicians and the care team, and educating patients, families, and staff on the Plan of Care and hospice philosophy.

Qualifications

• RN license is required in the state where you practice.
• Graduate of an accredited nursing program (certificate to BSN).
• Minimum 1 year of clinical nursing experience.
• Home health or hospice experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, and spiritual dimensions.
• Monitor patient condition and communicate changes to physicians and care teams.
• Educate patients, families, caregivers, and facility staff on the Plan of Care.
• Provide education on hospice philosophy, pain and symptom management to staff in various care settings.
• Offer compassionate support at the time of death and complete bereavement assessments.
