Qualifications

• Current RN licensure in the applicable state is required; associate degree preferred.
• CPR certification recommended (required for direct patient care in Georgia).
• Experience in hospice, home health, medical-surgical, or geriatrics/pediatrics is preferred.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with the care team to develop care plans.
• Maintain timely documentation and ensure plans reflect patient status.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations and admissions.
• Coordinate all aspects of patient care when serving as Care Manager, including scheduling and supervising staff.
• Respond to triage calls and participate in IDG meetings when in On-Call capacity.
