Job summary

AccentCare Inc. is seeking a dedicated Hospice After Hours Admission RN in Amarillo, TX. The role supports after-hours admissions with a strong emphasis on interdisciplinary collaboration, patient-centered care, and adherence to hospice regulations.

You will conduct comprehensive assessments, coordinate care plans, and provide education to patients and families while participating in on-call rotations. A valid RN license, willingness to travel within coverage area, and proficiency with EMR

Qualifications

• Current RN license in the applicable state is required.
• CPR certification is recommended (required for direct patient care in some states).
• Willingness to participate in on-call rotation and maintain reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with the care team to develop care plans.
• Maintain accurate documentation and coordinate care resources.
• Educate patients and caregivers, supporting transitions and on-call needs.
• Coordinate care management including scheduling and supervision where applicable.
• Respond to triage calls and participate in IDG meetings as directed.
