Job summary

AccentCare is seeking an experienced RN for after-hours hospice admissions in the North Dallas area. This full-time weekend position requires Friday 5p to Monday 8a coverage and involves collaborating with an interdisciplinary team to assess and plan patient care.

Qualified candidates hold an active RN license, prefer ADN, and have experience in hospice or related fields. Strong communication and EMR skills are essential for effective care coordination and on-call responsibilities.

Qualifications

Active RN licensure in the applicable state.
Associate degree in nursing preferred.
CPR certification recommended (required for direct patient care in some regions).
Experience in hospice, home health, medical-surgical, geriatrics or pediatrics preferred.

Responsibilities

Conduct comprehensive nursing assessments and collaborate with patients, families, physicians and IDG to develop, implement, and evaluate care plans.
Maintain timely and accurate documentation and ensure care plans reflect patient status.
Provide education and emotional support to patients and caregivers; participate in on-call rotations and admissions.
Coordinate all aspects of patient care including scheduling, medication management, and supervision of LPNs/LVNs and Hospice Aides; participate in IDG meetings.
Respond to triage calls, perform visits and assessments, and participate in IDG meetings and family conferences as directed.
