Job summary

AccentCare is seeking an experienced RN for after-hours hospice admissions in the North Dallas area. This full-time role requires participation in an on-call rotation, with a Monday-Thursday schedule and on-site responsibilities supporting patients and families in hospice care.

The position emphasizes comprehensive nursing assessments, care plan development, and strong teamwork with IDG. A valid RN license and relevant hospice/home health experience are preferred.

Qualifications

• Must hold active RN licensure in the applicable state; associate degree preferred.
• CPR certification recommended (required for direct patient care in Georgia).
• Experience in hospice, home health, medical-surgical, geriatrics, or pediatrics is valued.
• Willingness to participate in on-call rotation and maintain transportation and insurance.
• Strong communication and organizational skills; proficient with EMR and MS Office.
• Ability to collaborate within an interdisciplinary team.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized care plans.
• Maintain timely documentation and coordinate with care teams for optimal resource use.
• Provide education and emotional support to patients and caregivers; participate in on-call and admissions.
• If Care Manager: coordinate scheduling, medication mgmt, IDG meetings, and supervision of staff.
• Respond to triage calls and support care transitions as directed; participate in IDG meetings.
