Job summary

Hospice of San Joaquin in Stockton, CA is seeking an experienced RN - Admissions to join our team. You will evaluate patients for Medicare Hospice Benefit, perform initial assessments, and work with the IDT to plan and deliver care in a fast-paced setting.

Qualifications

• Current California RN license.
• Two years RN experience; hospice experience preferred.
• Ability to set priorities and work independently.
• Strong nursing judgment and knowledge of hospice care.
• Effective written and verbal communication skills.
• Willingness to drive as required.

Responsibilities

• Evaluate patients for appropriateness for Hospice Medicare Benefit.
• Provide skilled nursing assessment and direct patient care.
• Collaborate with IDT to develop and update the Plan of Care.
• Coordinate with social services, home health aides, and contracted providers.
• Maintain comprehensive patient records in compliance with standards.
• Support families and coordinate with team during bereavement planning.
• Demonstrate advanced nursing practice and advocate for patient safety.
