Job summary

Bristol Hospice in Stockton, CA is seeking a compassionate Registered Nurse (RN) with strong assessment skills to join our hospice team in Stockton. This PRN hourly role involves comprehensive patient and family assessments and collaboration with an interdisciplinary team to deliver high-quality care.

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of home care experience preferred.
• Must have a current licensure in the state.
• Must understand the philosophy of hospice care and the needs of the terminally ill.
• Must be flexible in work hours.
• Must demonstrate willingness to maintain knowledge of information systems and software programs.
• Ability to travel throughout the Bristol Hospice Service Area.

Responsibilities

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Develop a care plan that establishes goals based on nursing diagnosis and revise as needed.
