Job summary

Moments Hospice is seeking an on-call Registered Nurse to organize and direct hospice care for after-hours and weekend coverage. This full-time, 7 days on / 7 days off position includes evening/nights shifts and travel within service area.

Responsibilities include comprehensive patient assessments, care planning, and coordination with physicians and families. A valid RN license, strong clinical skills, and a reliable vehicle are required.

Qualifications

• Licensed Registered Nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Perform comprehensive initial and ongoing patient assessments for hospice needs.
• Respond to after-hours calls, triage, and coordinate weekend admissions.
• Provide professional nursing care using the nursing process.
• Update and revise care plans as patient status changes.
• Deliver medications and treatments as prescribed.
