Moments Hospice is seeking an experienced registered nurse to cover after-hours and weekend on-call shifts. The role involves direct patient care in home settings, with a focus on symptom management and urgent assessments.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Complete initial, comprehensive and ongoing assessments to determine hospice needs for after-hours calls.
• Respond to calls from Triage during shifts; perform visits to patients with changing conditions.
• Provide professional nursing care using the nursing process.
• Write and initiate the plan of care; re-evaluate patient and family needs.
• Initiate and revise plans as patient status changes and needs evolve.
• Use health data to determine nursing diagnosis and set goals with family involvement.
• Administer medications and treatments as prescribed; educate patient and family.
• Maintain accurate clinical notes on patient condition and care provided.
• Communicate with physicians about patient needs and order changes.
• Coordinate care with community health professionals; teach self-care to patient/family.
• Ensure a safe patient environment and continuity of care; participate in quality initiatives.
