Job summary

Moments Hospice seeks an on-call Registered Nurse to organize and direct hospice care within patients’ homes and communities. You will cover nights and weekends on a 7 days on/7 days off schedule, with after-hours calls, comprehensive assessments, and plan-of-care management.

Qualified candidates are licensed RNs with EMR experience, strong planning and communication skills, and a valid driver's license with reliable vehicle.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Performs comprehensive assessments of patient and family to determine hospice needs for after-hours care.
• Responds to calls from Triage during assigned shifts and makes visits to patients.
• Delivers professional nursing care using the nursing process.
• Creates and revises the plan of care as patient needs change.
• Documents and communicates patient status and care plans with physicians and team.
