Job summary

Moments Hospice is seeking an on-call registered nurse to cover nights and weekends, delivering compassionate home-based hospice care. You will assess patient and family needs, coordinate with physicians, and ensure safe, effective care in a community setting.

This full-time position features a 7 days on/7 days off schedule, a company car with fuel and insurance, and a benefits package including health, dental, and vision insurance, generous PTO, and 401(k) matching.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR).
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Performs initial and ongoing patient assessments to determine hospice needs.
• Responds to after-hours calls and visits patients with changing conditions.
• Provides nursing care using the nursing process and plan of care.
• Writes and revises care plans as patient needs change.
• Administers medications and treatments as prescribed.
